表格贊助
Sponsorship form (Donate by Deposit or Cheque)
填妥贊助表格後，請連同銀行存款收據副本電郵至 [email protected] 或 連同劃線支票郵寄至新界葵涌葵昌路50號葵昌中心7樓703室。
Please complete and send your Sponsorship Form together with the deposit receipt to TGE by email: [email protected] or send cheque to The Green Earth. Address: Room 703, 7/F, Kwai Cheong Centre, No.50 Kwai Cheong Road, Kwai Chung, Hong Kong
若您於報名時未有提交所有捐款人姓名。你亦可以透過填寫贊助表格，補交最低籌款額或/及額外捐款的捐款人姓名，讓我們為你發出免稅收據。
If you have not given us the entire name list of the donors at the time of registration for your minimum donations and/or extra donations, you can fill out the form and allow us to issue the receipts to your donors.
*凡捐款港幣100元或以上，可獲發免稅收據。請在捐款/贊助表格上清楚填寫捐款/贊助人資料。
Donation receipt will be issued for donation of HK$100 or above. Please provide information of the donor/sponsor clearly in the form for TGE to make receipts.
